Welcome to our dental office. Thank you for allowing us to help with your dental care. A Welcome Packet will be mailed to you in advance of your appointment if you have called a week or more ahead of your appointment. This Welcome Packet is available at the office for pickup if you prefer to fill paperwork out here or you can print the needed forms from this page and complete them at home and bring them with you to your appointment.
If you are transferring here from another dental office, please request they send the current x-rays to us. If you would prefer us to call, we will be glad to, but will need you to complete the “Request for Transfer of Records”. This form is required to comply with HIPAA (Health Insurance Portability and Accountability Act.
If you are unable to complete the enclosed paperwork, or are unable to download the forms online and complete them, please arrive 15 or more minutes early to complete the necessary forms for our office. If you would like help completing the forms and this is your first visit to this office, we request you arrive 30 minutes early, especially if you have a complex health history or other needs. We take considerable effort to begin our appointments promptly. If you may be late for your appointment, please let us know in advance to ensure we can best meet your needs.
